If you are interested in attending In-House, Outside, or Computer training, please do the following:

* Complete this form. * If request is for outside training, attach a copy of the training brochure. * Submit to your division director or department head for approval.

Once this request is approved by the appropriate division director or department head, the Human Resources and Staff Development Department will process it. You will receive notification of your registration. Requests for computer training will be forwarded to the IR Trainer for further processing.
